I am hoping someone will be able to give me advice on how to address a gap in CHIP implementation for our county. Due to unforeseen circumstances with our action team facilitator, we do not yet have our next CHIP completed and the current CHIP is expiring on December 31, 2018. How would you recommend to address this? The idea of adding an addendum to the current CHIP stating that the plan will instead run through April 1, 2019 was suggested. Other than that, we are open to any ideas that folks have or would be willing to share from personal experience.
